VFC Re-Enrollment Process

 Purpose

The Idaho Immunization Program (IIP) administers the federal Vaccines for Children (VFC) program in Idaho. Providers must be enrolled with the IIP to receive the vaccine. Providers already enrolled with the Idaho Immunization Program (IIP) are required to re-enroll annually.  The provider re-enrollment process will be conducted online through IRIS beginning in 2018.

Note: Healthcare providers wishing to enroll for the first time can begin by contacting the IIP by phone (208-334-5931) or email (IIP@dhw.idaho.gov).

 Functionality

The enrollment process forms can be accessed using the left menu navigation in IRIS. The Provider Agreement, Provider Profile, and Brand Choice pages can be accessed individually to make changes outside of Re-enrollment period or accessed individually (along with the final step Review and Sign) to complete the VFC Re-enrollment during the VFC Re-enrollment period.

The staff responsible for VFC re-enrollment verifies the existing information as well as make changes, adds information as needed by going through the forms Provider Agreement, Provider Profile, and Brand Choice. Once these forms are completed and saved with all the required information, the staff will have the option to submit the VFC re-enrollment forms to the Medical Director or Equivalent using the 'Submit' button available in each of these forms. The submit but will be unavailable once the forms are submitted to Medical Director or Equivalent.

The Medical Director or Equivalent will receive an e-mail when the staff from the facility submits the VFC Re-enrollment forms. The Medical Director or Equivalent then can review the forms (and change if needed), sign, print the re-enrollment packet and then submit to IIP which completes the VFC re-enrollment process.

The VFC Re-enrollment forms have a step progress bar at the top of the page which shows the overall progress of the VFC enrollment process. The below screen capture, for example, shows that the Provider Agreement and Provider Profile are completed and saved and Brand Choice is still not completed. Once this is completed, saved and submitted to Medical Director or Equivalent, the steps 3 and 4 will turn to green. Once the Medical Director or Equivalent reviews, signs and submits to IIP, the step 5 will turn to green which completes the VFC re-enrollment process.
