Request Vaccine Screen

Purpose

This Screen defines the changes needed to process that will allow a provider to Order against vaccines they have Pre-Booked at an earlier date. Changes covered here include changes to a new “Request Vaccine” screen (similar to how the current Manage Orders functionality works).

Information Provided

Provider information section - This section displays information that is specific to the for a provider organization as specified in the table below.

Field Name	Description
VFC PIN	This information displays at the top of the screen, this is the VFC PIN for the organization logged in.
Initiating Organization	This information displays the organization which is logged in.
Initiating User	Display the name of the delivery contact for the provider organization who submitted the order.
Phone Number	Phone number tied to provider organization logged in.
Fax Number	Displays the Fax number tied to provider organization logged in.
Request Date	This information displays the date order started.
Primary VFC Coordinator	Displays the name of the Primary VFC Coordinator for the provider organization logged in.
Delivery Address	Displays the Delivery Address for the provider organization logged in.
Vaccine Delivery Hours	Display the Delivery Days and Hours for the provider organization logged in.
Special Instruction	Displays the Special Delivery Instructions if any.

Request Vaccine section - This section will display the prebookings information for event selected as described in the table below.

Column	Description
#Doses	This column display the quantity of doses that can be requested by the provider.
Trade Name	This column displays the Trade name of the vaccine request.
Packaging	This column displays the Packaging information for the vaccine.
Manufacturer	This column displays the manufacturers information of the vaccine request .
Min Order Quantity	This column display the smallest order that can be placed based on NDC screen calculations (units per package x doses per unit).
Type	This column displays the type of vaccine e.g. VFC, Adult and Pandemic.
Doses Prebooked	This column displays the no of doses prebooked in the prebook screen.
Current Doses Allocated	This column displays the amount of doses allocated to a provider.
Doses Remaining	This column displays the difference between quantity Prebooked and quantity Allocated for this NDC and associated Event

Command Button Functionality

Command Button	Description
	Clicking on the Submit button, submits the vaccine request order for the number of doses indicated by the user.
	Clicking on the Accept All button, Submits the order for the entire allocated amount on the screen without having to fill in the # of Doses fields.
	Clicking on the Hold All button, holds the allocation for a [X days is based on pre-set window].
	Clicking on the Decline All button, declines the entire allocated amount for the current rows and places. It will also decline the other allocations that were not requested before, for the same vaccine made by the provider.
	Clicking the Cancel button, will display the previous screen the user was on.
