Purpose |
The Create a Request - Funding Source Public screen is used to initiate a request to record vaccine wastage or return vaccines to the distributor. The Create a Request screen can be reached from the Manage Wastage and Returns screen. Note: The Create a Request screen initiates wastage and returns requests only for public-funded inventory.

The Create a Request-Funding Source Public screen is broken into four sections: Creating a Request, VFC Profile, Wastage/Return Information and Vaccine Wastage or Return Tables.
Create a Request
The Create a Request-Funding Source Public section displays non-editable information, relevant to the specific Request.

Status |
Status of the Request. When a Request is created, the default status is Pending. |
Submit Date |
Date when the Request was created. |
Submitted By |
User who created the Request. When you create a Request, your name should be listed in this field. |
Phone Number |
Phone Number of the user who created the Request. When you create a Request, your phone number should be listed in this field. |
Email address of the user who created the Request. When you create a Request, your email address should be listed in this field. |
Contact Information
The Contact Information section will list information from your organization's VFC profile such as VFC Pin and delivery address. The delivery contact person can be a Primary Vaccine Coordinator or a Backup Vaccine Coordinator or a Main Contact if the Primary Vaccine Coordinator and Backup Vaccine Coordinator are not available. If the information in this section is not correct, please contact your administrator.
Note: The contact person selected when creating a Request will be exported as the contact when the Wastage/Return is exported to VTrckS.

Wastage/Return Information
The Wastage/Return Information section has radio buttons that allow you to decide whether to record a wastage or request a vaccine return.
If you click the Record a Wastage radio button, the screen will turn light blue, the Vaccine Wastage table will appear and you will need to perform the following:
Fill in the Transaction Date field. The Transaction Date is the actual date when the vaccine wastage was processed.
Select an option from the Detail dropdown. If you select the 'Other' option, you need to fill in the Explanation field.
If you click the Request a Return radio button, the screen will turn light brown, the Vaccine Return table will appear and you will need to perform the following:
Select an option from the Return Type dropdown - either Return Only or Replacement.
Select an option from the Detail dropdown. If you select the 'Other' option, you need to fill in the Explanation field.
Vaccine Wastage or Return Tables
The "Show" radio buttons allow you to view either Active, Inactive, Expired or All vaccines. Inactive and Expired vaccines are marked in red.

Functionality |
Click either the Record a Wastage or Request a Return radio button.
If you click the Record a Wastage radio button, fill in the Transaction Date field and select an option in the Detail dropdown.
If you click the Request a Return radio button, select an option from the Return Type and Detail dropdowns.
If you select the 'Other' option from the Detail dropdown, you need to enter comments in the Explanation field.
In the Vaccine table, click either the Active, Inactive, Expired or All radio button.
Fill in the Quantity to be Wasted or Quantity to be Returned field for at least one vaccine in the Vaccine table.
Click Save & Submit to submit the Request to the state, or select Return to List to return to the Manage Wastage and Returns screen. Note: After you click the Save & Submit button, you will not be able to click the Record a Wastage or Request a Return radio button. You will not be able to turn a wastage Request into a return Request (and vice versa).
