Compromised Vaccine Recall

 Purpose

This element of IRIS will be used to track and communicate vaccines that have been compromised. As such, users will be able to request a listing of clients active to a specific organization to which they administered immunizations from a particular vaccine group, vaccine lot or trade name, during a particular time period.  Organizations may then generate as output from this request client contact lists or custom mailing letters.

Functionality

Users will be able to generate patient lists based on dates immunizations were administered to specific patients, from specific inventory/vaccine lots, specific to an organization.  System Managers will be able to generate statewide patient lists.  Upon generation of patient lists, outputs from the Compromised Vaccine Recall Output Options screen can be generated:

 

To submit a Compromised Vaccine Recall Request:

  1. Select the Vaccine Recall link under the Reports Group menu list

  2. Add a Date Range. Use MM/DD/YYYY format

  3. Select Lot #(s) from the Lot Numbers from Inventory pick list - Click Add button

  4. OR Lot #(s) can be entered manually in the Key-In Lot Number field - Click Add button

  5. Select Generate button

Option

Description

Vaccination Date Range

Establishes a date range for vaccines administered. Use MM/DD/YYYY format. User can also add dates by using the calendar icon.

Lot Numbers from Inventory

This section allow user to select, from a pick list, Lot number(s) from existing inventories that are associated with recall. Multiple Lots can be selected by holding down the CTRL key.

Key-In Lot Number

User can manually enter lot number(s) associated with recall.

Specify How to Sort the Report Data

This section provides user with the option to sort recall data by selecting up to 4 different types of criteria. The default report will sort by Last Name and  then by First Name.

Add Button

Clicking this button moves Lot numbers to the Selected Lot Numbers field

Remove Button

This button removes selected Lot numbers from the Selected Lot Numbers field

Generate Button

This button generates user's recall request based on request parameters entered.

Cancel Button

This button cancels user's recall request and returns to previous screen.

Compromised Vaccine Recall Output Options

To generate a Standard Compromised Vaccine Recall Letter from the Compromised Vaccine Recall Output Options:

    1. Add the ‘reason for recall’ in the FREE TEXT box. This field allow the user to explain in the content of the letter the reason why a patient is being notified.

    2. Report Name – The report name for the report can be renamed if a user would like to distinguish their report from other reports. The default name for this report is “Vaccine Recall Letter.” This step is Optional.

    3. Select the blue ‘Recall Letter’ Link. This will generate recall letters for patients who have been identified for recall based on Lot # criteria.

    4. Selecting this link will navigate user to the Compromised Vaccine Recall Output Status screen.

To generate Mailing Labels from the Compromised Vaccine Recall Output Options:

    1. Report Name – The report name for the report can be renamed if a user would like to distinguish their report from other reports. The default name for this report is “Vaccine Recall Label.” This step is Optional.

    2. Select the blue ‘Mailing Labels’ Link. This will generate mailing labels for patients who have been identified for recall based on Lot # criteria.

    3. Selecting this link will navigate user to the Compromised Vaccine Recall Output Status screen.

Recall Letter

Generates a Standard recall  letter for patients who have met the Vaccine Lot criteria established during the Compromised Vaccine Recall Request

Recall Necessary Because...

User must enter reason for recall. Content entered in this field will appear in body of patient's recall letter.

Mailing Labels

Mailing Labels generated based on recall letters produced.

Patient Information Spreadsheet

Excel spreadsheet provides detail information for patients eligible for recall.

Report Name

This field allows user to rename and identify their output report, i.e., Recall Letter, Mailing Labels, Patient Information Spreadsheet

This button will cancel action and return to previous page.
