State Prebook Report Request Screen |
Purpose |
The State Prebook Report screen allows State users to create reports and monitor vaccine prebook and order requests
Report Criteria |
To produce a State Prebook report, select the desired option. Listed below are the descriptions for each option.
By Prebook Event: Select to generate report for prebook activity related to a Event
By Date Range: Select to generate report for prebook activity related to selected Date Type. The following options are required to generate a report by date range:

Date Type |
Select the Date Type criteria: Prebook Date, Allocation Date, Request Date or Date Shipped. |
From and To Dates |
Enter the report from and to date range in MM/DD/YYYY format. |
For By Prebook Event or By Date Range, select additional report filter items :

Select Organizations |
The All VFC Organizations option is selected by default and will return all Organizations with the VFC status "Active", "Suspended", "Pending", and "Noncompliant". |
Organization Selection |
Selecting Individual Organization allows the user to select organizations to be included in the report. |
Select Vaccine Type |
By default, Vaccine Type will contain all Vaccine Type values that exist on the Edit NDC Information screen. Values are VFC, Adult, and Pandemic. |
Select Report Type |
User can select from 3 Report Type options: Summary, Detailed or Both. |
