The request new patient form screen is used to display and print a blank Vaccine Administration Form. To generate this form, select Forms at the top of the screen.  This form may then be filled in with the necessary information for a patient who does not currently exist within the system.

• Functionality

Click the Forms link at the top of the screen.  Then select the link titled Vaccine Administration Record corresponding to the language for which you would like the form generated in.
