Pre-Select Immunizations Screen

 Purpose

The Pre-Select Immunizations screen is used to prepare or pre-populate immunization information to be recorded and saved on the Record Immunizations screen.

 Information Provided

Below the patient information is a section used for selecting default information for new immunizations. This section contains information that can be chosen which will pre-populate the final immunization records with the selected information for all new immunizations selected.

Field Name

Description

 Organization Site 

This field displays the site within the provider organization who's inventory will be drawn from and is listed within the 'Immunizations' frame for new immunizations. Choose a site by selecting the desired site name from the pick list. When a new site name is clicked, the Pre-select Immunizations screen will automatically reload with the selected site's inventory displayed in the grid.

 Ordering Authority 

This field names the clinician who has the ordering authority to let the patient have the selected immunizations. Select an ordering authority by choosing a clinician name from the pick list.  Clinicians found within the drop down list are configured by your administrator.

 Administered By 

This field names the clinician who actually gave the selected immunizations to the patient. Select an administering clinician by choosing a clinician name from the pick list.  Clinicians found within the drop down list are configured by your administrator.

 Date Administered 

To pre-populate all the new immunizations being recorded with the same administration date, type a date in the format MM/DD/CCYY into the text box. Or, click on the Calendar icon to set the date. *Note*

VFC Eligibility

This field contains a drop down box of the VFC eligibility descriptions. The VFC eligibility is recorded for each immunization on the day it was given. VFC eligibility is required for patients <19. For patients > 19, the eligibility will default to Ineligible regardless of the selection made.

Insurance Provider

This field contains a drop down box of the insurance providers stored within the IRIS. The Insurance Provider is recorded for each immunization on the day it was given. Select 'Other Insurance' and contact the IRIS Help Desk if you do not see your expected insurance provider. Insurance Provider is required for VFC ineligible patients who are <19 on immunization date.

Following the default information is the pre-select information for entering immunizations. There are two ways of entering immunizations. The information entered into these columns will help build the final immunization records to be saved on the database.

The column labeled From IRIS Inv means the provider is using the IRIS Inventory module and the inventory will be decremented accordingly. Boxes will only display in this column when the provider has inventory set up in IRIS inventory. Users will select the check boxes in the “From IRIS Inv” column for the vaccines that were given from IRIS inventory.

The column labeled Hist # allows the user to enter the number of historical immunizations (not owned) to be entered within the subsequent screen.

 Functionality

Button

Description

When the Ok button is clicked, the Record Immunizations screen is displayed containing all the immunization information selected from the Pre-select Immunizations screen.

When the Cancel button is clicked, any changes and/or selections made are lost, and the Immunization History screen is displayed.

If it is necessary to view the selected site's inventory from a past date, enter that date here and click the Activate Expired button.  This will make currently expired vaccine lots available for the administration of immunizations on dates occurring prior to the vaccine lots expiration date.

Click the Unselect All button to clear all check boxes and text boxes.

 Notes

If the date administered field is left blank for new immunizations on this screen, today's date will be entered automatically in the Record Immunizations screen.
