Trade Name

This information at the top of the screen displays the Trade Name of the NDC that is being entered.

Vaccine

This information at the top of the screen displays the Vaccine of the Trade Name of the NDC that is being entered.

CVX

This information at the top of the screen displays the CVX Code of the Vaccine of the Trade Name of the NDC that is being entered.

NDC Number

NDC Number is a required text field.  Specify the NDC Number being entered in this text box.

Packaging Description

Packaging Description is a required text field.  Specify the Packaging Description of the NDC being entered.

 Dummy NDC

The dummy NDC radio button is a required field select the value ’Yes’ or 'No'. Defaults to 'No'.

Dose Size

Dose Size is required to be selected from the Dose Size pick list.  The pick list displays all available Dose Sizes.

Units Per Package

Units Per Package is required to be selected from the Units Per Package pick list.  The pick list displays all available units per package for NDC Codes.

Doses Per Unit

Doses Per Unit is required to be selected from the Doses Per Package pick list.  The pick list displays all available doses per unit available for a unit.

Manufacturer

This pick list displays all the available manufacturers for the Trade Name associated with the NDC.

Direct Ship

Select whether the vaccine is a direct ship vaccine or not.

Orderable

Selecting if the vaccine is orderable for Pediatrics, Adults and Pandemic is required. Select 'Yes', 'No', or 'Unavailable' from the pick list.

Available to Request

Choose if a the vaccine can be prebooked. Select 'Yes', 'No', or 'Unavailable' from the pick list.

Type

This is a required text field. Select the values ‘VFC’, ‘Adult’, ‘Pandemic’.

Link to other NDCs

This is a required text field. Select the values  ‘Yes’ or ‘No’.

Linked NDCs

This is a required text field. Select the  linked NDC from the list box.

   Days to Auto-Return to Pool

This text box displays the number of days after the doses will automatically be returned to the  inventory.

Cost Per Dose

Accepts the pediatric cost per dose.

Adult Cost Per Dose

Accepts the adult cost per dose. This needs to be entered only if it is different from the pediatric cost per dose

Effective Date

Accepts the effective date of the cost entered. Defaults to system date.

VFC Contract

Accepts the VFC contact number.

Clicking the > button will add each  value selected from the left Select box to right-hand box.

Clicking the >> button will add all values selected from the left Select box to right-hand box.

Clicking the < button will remove each value from the right-list box to the left- list box.

Clicking the <<  button will remove all the values selected from the right-list box to the left- list box.

Clicking the  Add New Cost button will add the entered cost and contract information for the NDC.

Clicking the Save button will save the Entered NDC information if filled out correctly and display the Manage NDC screen.

Clicking the Cancel button will display the Manage NDC screen.
