Student Immunization History Screen |
Purpose |
The Student Immunization History screen displays a students immunization history and provides immunization recommendations based on the assigned tracking schedule. From this screen, you are also able to select and add the student to a specific student list. This will allow you to run reports for that student.
Information Provided |
The Student Immunization History screen displays a very large portion of the information held in the system database for the selected student. It is possible, and can be useful, to print this screen. Printing the information therein is possible directly from this screen or utilizing either the or
buttons. All three options require your internet browsers print screen function. If you do print screens, please note the following:
Print screen isn't a IRIS system function, but rather a function of your internet browser. If you use Print Screen it may print the web site address location on the bottom of the page, and this is a IRIS system security issue. This web site address is valid for one full day from the time of printing due to the fact that users must have the ability to leave the connection to the IRIS open. If you print screens and give them to parents or others, please do one of the following: 1) at a minimum, manually cut off the web address or 2) on your browser, go into File... Page Setup... and remove the footer data.
Student Information
The Student Immunization History screen displays a student information header at the top of the page. This header includes student name, selected tracking schedule, as well as other patient-distinct information.
Patient comments
The patient comments are visible at the top of the screen when you edit an immunization or when you add an immunization. When there is more than 1 comment just click on the up and down arrows to scroll through any available comments. When using the Print button on the immunization history page, the comments are displayed on separate lines in the Comments box.
Student Current Status
Student current Eligibility or Status will be five-fold.
Adequately Immunized
-All required vaccines are up to date. No student specific school report will be generated for a child with Adequately Immunized status
Conditionally Admitted/Attending
-Conditionally Admitted for School, the student will be included in the School Conditional Admission report; expiration date will default to the EARLIEST Recommended Date + 14 days for Vaccines Required for School Entry. For Vaccines Required for 7th or 12th Grade entry, the expiration date is the EARLIEST Past Due Date + 14 days.
-Conditionally Attending for Childcare, the student will be included in the Childcare Conditional Attendance report; expiration date will default to the EARLIEST due date of NEXT dose + 14 days.
Exempt
-The student must have a waiver (medical, philosophical or religious) on file with that specific School or Childcare facility. Each organization must retain their own waiver form on file for the child.
Incomplete
-The student will be included in School/Childcare Legal Notice report; deadline date will default to the current date + 14 days.
No Record
-The student will be included in School/Childcare Legal Notice report; deadline date will default to the current date + 14 days.
Incomplete Address Warning message
The below warning message will be displayed after Current Status when the student's address on file has missing Zip code and patient’s country is United States or one of the US Territories. After the Zip code is updated by editing the patient, this message will not be displayed.
Add Student to Report List
One main piece of functionality in this section is the 'Add to Report List' drop down box. With this drop down box you can select a list you want your student to be added to so you can run a report for that student. Here is what the drop down box looks like:
To add the student to a list click on the drop down box, then select your list from the available options. Then click the Add this Student to a Report List hyperlink. A message will display saying 'Student has been added to the selected list'. Note - Once a student has been added to a list the student cannot be added to the same list again until removed from that list. The student can be added to any other list's though.
The information contained in the header can also be used to confirm that you have located the correct student when you were brought to the Student Immunization History screen following a student search on the Find Student screen.
Student Immunization History
The Student Immunization History table lists all vaccinations the selected student has received to date. Immunizations listed in the table are ordered alphabetically first, then by 'Date Administered'. The table columns are defined as follows:

This column gives the actual day the patient was given the vaccine. |
Series |
This column denotes the sequence number within the immunization series (i.e. 1 of 3). |
Trade Name |
This column displays the trade name of the vaccination received. |
Dose |
This column displays the number of doses administered. |
Reaction |
This column displays whether any reactions were experienced by the student to that specific immunization. The entire row will be highlighted in red were a reaction present. Select the Yes hyperlink to view the specific reaction. |
Students Age on Student Immunization History Screen
The students age shows in a solid blue field between the student immunization history and before the recommendations. Here is an example of what it looks like:
Vaccines Required For School/Childcare Entry
The recommended vaccinations and corresponding dates for the selected student reside in a table titled Vaccines recommended By Selected Tracking Schedule at the top of the table. The vaccine group list appearing in this table represents all vaccinations included in the tracking schedule assigned to this student. The table columns are defined as follows:

This column contains check boxes which can be used to pre-select any of the tracking schedule's immunizations when adding immunizations to the selected student. The check box will be disabled for School/Childcare users, or if the tracking schedule's recommended vaccination is contraindicated, or if all required immunizations for that vaccine group have been received. |
Vaccine Group |
This column gives the recommended vaccine group name. Click on the vaccine hyperlink DTP/aP to view the "Explanation of Dose Status" popup for that particular vaccine recommendation. This popup window displays the tracking schedule rule applied to the selected recommendation for that series. |
Vaccine |
This column displays a vaccine name if a specific vaccine is recommended. The IRIS System Manager can choose to hide or display vaccine recommendations in a schedule |
Earliest Date |
This column displays dates which note the earliest date the selected patient could receive the corresponding immunization. |
Recommended Date |
This column displays dates which note the date that the selected patient is recommended to have the corresponding immunization. |
Past Due Date |
This column displays dates which note the date that the patient is past due for the corresponding immunization. |
The dates found in the Vaccines recommended by Selected Tracking Schedule table can be shaded with color for emphasis. The colors applied to the dates within the columns are defined as follows:

The color green will highlight dates in the Earliest Date, Recommended Date or Past Due Date column. Green shading indicates the at today’s date is equal to or past the highlighted date, and is before the next date milestone. Therefore the immunization is recommended to be administered now. |

The recommendations for vaccines now reflects whether a patient has exceeded the maximum age or maximum doses for a specific vaccine. For instance, if a patient has already reached the age of 5 and hasn't received any pneumococcal conjugate vaccine (Prevnar 13), then the recommendation for Pneumococcal at the bottom of the immunization history will show "Maximum Age Exceeded" and the patient will not show on reminder reports as needing that immunization. |

The 'Max Doses' label indicates the maximum number of doses has be given for the specified series. |

The 'Immunity Recorded for Vaccine Group' label specifies the patient comments include an indication that the patient has is verified immunity for the disease. If a patient has an immunity recorded, the vaccine does not need to be given. |

The 'Contraindicated' label specifies the patient comments include a condition that contra indicates the vaccine. If a patient has been contraindicated, then the vaccine should not be given. |

The 'Complete' label specifies the patient is complete for the corresponding vaccine. This means the patient will not display on reminder reports for this specific vaccine. |
Vaccines Required for 7th Grade Entry
The recommended vaccinations and corresponding dates for the selected student reside in a table entitled Vaccines Required for 7th Grade Entry. Students entering the 7th grade must have completed the vaccines required for School Entry and the Pertussis (Tdap) series. In addition the student must have received at least a valid dose of MeningACWY (MCV4) vaccine at age 10 years or older.

This column displays dates which note the date that the selected patient is recommended to have the corresponding immunization. The Recommended date will be whichever occurs first: the 11th birthday, or the August 1st of the year the student will enter the 7th grade. |
Past Due Date |
This column displays dates which note the date that the patient is past due for the corresponding immunization. The Past Due date will be August 16th of the year the student will enter the 7th grade. |
Vaccines Required for 12th Grade Entry
The recommended vaccinations and corresponding dates for the selected student reside in a table entitled Vaccines Required for 12th Grade Entry. Students entering the 12th grade must have completed the vaccines required for School Entry, the Pertussis (Tdap) series and adolescent MeningACWY (MCV4) series.

Past Due Date |
This column displays dates which note the date that the patient is past due for the corresponding immunization. The Past Due date will be the later of August 16th of the year the student will enter the 12th grade or the minimum interval from the previous dose. |

This link is used for adding the student to a student list to be used for reporting. First you must select your list you want the student added to from the 'Add to Report List' drop down box. Then click the link. A message will appear: "Student added to Selected List" indicating the save was successful. |

When the Add Immunization button is clicked will bring you to the Record Immunization screen for the patient. Note: This button is not available for School/Childcare Standard User roles. Standard User roles can only views Immunization History, cannot edit or add immunizations. |

When the Edit Patient button is clicked will bring you to Edit Patient screen. |
|
When the Reports button is clicked will bring you to the Reports Available for This Patient screen. You may generate Patient-specific reports. |

When the Add selected button is clicked will bring you to the Record Immunization screen for the patient. Note: This button is not available for School/Childcare School User roles. School User roles can only views Immunization History, cannot edit or add immunizations. |
Viewing the Schedule Explanation Screen |
The Schedule Explanation Screen is used to display the vaccination rule applied to a specific dose selected. This can aid in explaining why a dose was not valid or why a recommendation displays a particular date as examples. The window displays the table view of the schedule for the dose that was selected. For example, you are wondering why a dose of DTaP is marked as Not Valid. When you click on the vaccination date for that dose, a window pops up displaying the series for DTaP and includes a comment "Greater than the Maximum Age required for this dose" if this were the reason for being Not Valid. The selected dose in the series will be highlighted in yellow.
Viewing the Schedule Explanation for an Immunization
For a specific dose in the history click on the date administered hyperlink in blue. (12/10/2011).
A window will open displaying the series associated with the selected dose.
If the selected dose is part of a series, it will be highlighted in yellow.
If it is Not Valid, the reason for being Not Valid will be displayed.
If the dose is not part of a series in this schedule, a message indicating that the dose is not part of a series in this schedule will appear. Note: If this is the first dose given for a vaccine group and it is recorded prior to the minimum age, it will also give this message.
When done viewing the information in the window, click on the 'X' in the upper right hand corner to close the window and return to the IRIS.
Viewing the Schedule Explanation for a Recommendation
In the Vaccines Recommended by Selected Tracking Schedule section on the history page, click on the vaccine group name, such as DTP/aP.
The window will open displaying the schedule for the vaccine group.
The selected dose in the series will be highlighted in yellow. Note: If a patient had no immunizations in the series and was too old to complete, the first dose in the series would be highlighted.
When done viewing the information in the window, click on the 'X' in the upper right hand corner to close the window and return to the IRIS.
