Report/Student List Screen

 Purpose

The Report/Student List screen is used to view students who have been added to your school list, and to run report's for those students.  You can also remove a student from your list.  The screen is broke into two sections: Reports Available for School and Client List for School.

 Overview

This section provides three report's for your students within the list.  Click the hyperlink to generate the report.  A brief explanation of either report is found in the table below.  Click any of the links within the table for a more detailed explanation.  

Report Name

Description

Student List

This report displays the name and date of birth for each student on the list sorted alphabetically by last name.

Student Immunization History List

This report displays the name, date of birth and immunization history for each student on the list sorted alphabetically by last name.

Immunization Due

This report displays the name, date of birth and all immunization information for immunizations due for each student on the list sorted alphabetically by last name.

This section is for viewing student immunization history, and to remove a student from your list if you wish to do so. Information contained within the patient list table is described below.

Column Name

Description

Delete

If you wish to remove a student from your list you use the Delete column to do so. Place a checkmark in the box beside the student(s) you wish to remove. You can remove as many students as you like at one time by checking multiple students. Next click on the Delete button. Your selected student(s) should then be removed.

Last Name

This is the last name for the student. It is also used as a link back to the Student Immunization History screen. Click on this link and you will be taken to the history screen for the student.

First Name

This field displays the first name for the student.

Middle Name

This field displays the middle name for the student.

Birth Date

This field displays the date of birth for the student.

 Functionality

  1. Click on the report name for the report you want to run.

  2. Generally you will be taken to the Check School Report screen where you can view the status of your report selection. Note - The student list report will open directly without having to go through the status screen.

  1. Click on the Student List link to generate.

  2. View report and select back to button within your browser to return to previous screen.

  1. Click on the Student Immunization History List to generate.

  2. View status of report compilation from the Check School Report screen.  *Note*

  3. Upon report compilation completion select the link to view the report.  Link will be in the form of History Report 01-29-2011 12:22:24.

  1. Click on the Immunizations Due link to generate.

  1. Select the Vaccine Groups  

Choosing vaccine groups allows you to single out any given vaccines within the selected tracking schedule. If the patient is due for any of the selected vaccine groups, based on meeting the status criteria, then he or she will be included in the report outcome.

    1. Within the 'Select the Vaccine Group(s) ...' section, select the 'Use All Vaccine Groups' option, or select the 'Use Vaccine Groups Selected' option.

    2. If the 'Use Vaccine Groups Selected' option is selected, choose the vaccine groups to include. Scroll though the vaccine group list and either double-click on the desired vaccine group name, or click on the vaccine group name and then click the Add> button. All selected vaccine groups will appear in the right listbox in green font. *Note*

  1. Select the Status Criteria

Status criteria can be selected as either Currently Due or Overdue or Due in the Next Number of Months.  Selection of one or the other is required.  Currently Due or Overdue will be selected by default.

Date Criteria

When this radio button is selected patients to be included within the report will be those due or overdue for an immunization.  Specifically, patients included will meet two criteria:

  1. Patient is at or has passed their recommended date for an immunization.  Selected Vaccine Group criteria will be applied in this determination.  For instance were DTaP the only vaccine group selected, patients included within the report would only be those recommended for a DTaP immunization, and all other vaccine group recommendations would not be compared against.

  2. The second criteria is that the patient can still receive the applicable immunizations and have them considered valid by the IRIS.  For instance, a patient may have passed their recommended date for an immunization, but were they passed the age requirement for which they could receive that immunization, they would be excluded.  

Recommended dates occurring on the current date will be included.

Due in the Next Number of Months

When this radio button is selected patients to be included within the report will be those becoming due for an immunization within the entered number of months.  They will meet the following criteria:

  1. Patients to be included will be those having a recommended date occurring within the number of months entered.  Consistent with Currently Due or Overdue behavior, Selected Vaccine Group criteria will be applied in this determination.  For instance were DTaP the only vaccine group selected, patients included within the report would only be those becoming recommended for a DTaP immunization within the time frame indicated, and all other vaccine group recommendations would not be compared against.

  2. Recommended dates on the current date will not be included.

  3. Recommended dates occurring on the day after the current will be included.

  4. Recommended dates occurring on the final date, of the months entered boundary, will be included.  For instance, if the report is ran on January 31st, for Due in the Next 2 Months, patients having a recommended date on March 31st will be included, but those having a recommended date on April 1st would not be included.

No more than 6 months into the future can be searched.

  1. Edit the Report List with in the text box next to "Client List for:".

  2. Click on the Rename button and you will be prompted with a confirmation to rename.

  3. Click OK will rename the list. Click Cancel will cancel the rename.

  1. Place a checkmark in the delete column next to the student you wish to remove. Remember: you can select and delete multiple students at one time.

  2. Click on the Delete button and you will be prompted with a confirmation to delete.

  3. Once you have confirmed a message will display: "The list has been updated successfully.", and your student(s) should be removed.

 Notes

Check School Reports Screen Note: You do not have to remain on this screen while the reports are running, however only one report will be able to be generated at any one time. To get back to the status screen for your reports click on the 'check school report' link in the menu panel on your left.

Selecting the Vaccine Groups Note: To remove any vaccine groups from the report criteria, either double-click the selected vaccine group name in the right list box, or click on the selected vaccine group name from the right list box and then click the <Remove button.
