Introduction |
The Legal Notice Report exists so that a school/childcare user is able to generate the legal notice and address labels for students who are not compliant with immunization requirements for school/childcare attendance according to state law. This report would include students that have an eligibility status of 'Incomplete' or 'No Record'. Read further to find out how to use criteria to improve the results of your report.
The Conditional Admission Report exists so that a school user is able to generate the Conditional Admission letter and address labels for students who are not compliant according to state law but have Conditional Admission eligibility to attend school until they are due for their next immunization. Read further to find out how to use criteria to improve the results of your report.
The Conditional Attendance Report exists so that a childcare user is able to generate the Conditional Attendance and address labels for children who are not compliant according to state law but have Conditional Attendance eligibility to attend a childcare facility until they are due for their next immunization.

The Legal Notice Report / Conditional Admission / Conditional Attendance reports are using same report request screen used to select and submit the student, date, and sort order criteria used to determine which students must receive a Legal Notice or Conditional Admission/Conditional Attendance letter.

Selecting a students Population
Note: The entire student population selected will be evaluated, but only students in need of a legal notice (not compliant eligibility status of No Record or Incomplete) will be returned in the legal notice results. If generating a Conditional Admission or Conditional Attendance report, only students with eligibility status of Conditional Admission/Conditional Attendance will be returned.

Students associated with your organization |
Selecting this option will return students in any of your school lists that meet the report criteria. This is considered normal reporting. |
Students with a specific graduation year |
Selecting this option will return all students with the graduation year specified in any of your school lists that meet the report criteria. |
Students associated to a specific school list |
Selecting this option will allow you to select one or more of your school lists. All students will be evaluated, but only students within the selected list that meet the selected report criteria will be returned. |
Selecting Age or Birth Date Range

Using the default selection, all students in population, your report will use the entire students base you selected. |
Birth Date Range |
Selecting an age range will narrow your results to only contain students who are between the indicated birth dates and are part of you population. |

Selecting an age range will narrow your results to only contain students who are between the indicated ages and are part of you population. |
Selecting Language Options
Choosing a language option gives you the flexibility to generate the reports all in one language, or to use each students' primary language according to their IRIS record.

Selecting this option will generate the letter for the students in the language specified on their primary address. |
Use One Language for all Letters |
Selecting this option will give you the opportunity to then select either English, or Spanish. Then, all notices will generate in the selected language. |
Selecting a Deadline Date
Selecting a deadline date is required for running the reports. Default deadline date is current date + 14 days. The deadline date will appear on the Legal Notice as indicating the deadline for the information to be returned to the school. The expiration date on Conditional Admission/Attendance Reports e is adjusted on the individual student letters to be 14 days after the next immunization is due. This indicates the last date the student will be eligible for attending school or childcare.
Selecting Sort Order
The Sort Order will determine the sequence which the students are listed for the output.
Within the 'Specify How to Sort the Report Data ...' section, choose the primary information field to sort the report on by selecting the information field name from the 'Sort 1st By' pick list.
Then choose how to sort the information field selected in step one by selecting Ascending or Descending from the second 'Sort 1st By' pick list.
If the sort order is not specified, the system will default to ordering the report results first by last name ascending and second by first name ascending.
Generating Labels
Click the box to generate labels if you wish to have a mailing label for each notice in the report. The labels will be formatted to print on Avery standard mailing labels.
Naming the Report
You can assign a name to the current report you are running, which will make it more distinguishable in your list of reports.

When the Generate button is clicked, a query executes in the database according to the report criteria selected. Depending on the number of students associated with the provider organization that submitted the report request, the query may take several minutes to complete. After clicking on Generate, you will be redirected to the check legal notice screen for Legal Notice report, check conditional admission screen for Conditional Admission report, and check conditional attendance screen for Conditional Attendance report where you can monitor the progress of the report query. *Note* |

Report Criteria Submission Note
After submitting your report by clicking on the Generate button you can work in any area of the system or logout of IRIS while your reports are generating without interrupting your report or causing system problems. To check on the status of your report click on the check legal notice link in the menu panel for Legal Notice report, click on the check conditional admission link in the menu panel for Conditional Admission report, click on the check conditional attendance link in the menu panel for Conditional Attendance report
