Patient Address Information Section |
Purpose |
The Patient Address section is used to manage a patient's address and phone number. You have the ability to make corrections to an existing address or record a new address in case the patient has moved. You may also view the patient's address history from this section.
Make Address Correction |
To correct a patient's current address, follow these steps:
Click the Manage Patient menu item in the menu panel on left-hand side of the page.
Use the Patient search screen to search for and select your patient. Click on the Address Information section header.
Make the correction directly to the current address which displays for the patient. Do not change or edit the start date.
Click the Save button. The system saves your correction. You may verify this if you go to View the Address History
Record Patient Move |
If the patient has moved, you may want to record a new address. To do so, follow these steps:
Click the Manage Patient menu item in the menu panel on left-hand side of the page.
Use the Patient search screen to search for and select your patient. Click on the Address Information section header.
Enter a start date for when the new address the patient moved to became effective.
Enter the new address information, overwriting the values for the current address.
Click the Save button. The system saves your correction. You may verify this if you go to View the Address History
View Address History |
To view the patient's address history, follow these steps:
Click the Manage Patient menu item in the menu panel on left-hand side of the page.
Use the Patient search screen to search for and select your patient. Click on the Address Information section header.
Click the View Patient Address History hyperlink.
The system displays a listing of all the addresses the patient has ever lived at and the start date for when each address became effective.
Click the X in the upper-right hand corner of the View Address History window in order to close it.

Zip |
The patient's 5-digit postal zip code. Zip code is required when Country is United States or a US Territory. One significant feature of this field is that you may enter any Idaho zip code, click Save and the system will automatically populate the city, state and county fields for you. Please note that you must leave the city and county fields blank in order for this feature to work. |
State |
The patient's state. When the state is changed, the system will automatically change the country based on the state selected. State is required only when Country is United States or a US Territory. The selected state will not be saved when country is not one of United States or a US Territory. |
Country |
The patient's country of residence. The state will be automatically changed when country selected is one of the US Territories. |

Undeliverable Address |
Check this box if the current address showing for this patient is no longer valid and no new address is known for the patient. Checking this box prevents the system from sending reminder recall letters to the patient. |
Start Date |
The start date indicates when the patient started to live at the address. |
Street Address |
The first line of the street address for the patient. |
Other Address |
The second line of the street address information for the patient, which may include apartment, room, or suite numbers. Note: If you choose to enter apartment information on this line, it is important to precede it with the Apt abbreviation. For example: 1520 Main St Apt 455 |
P.O. Box |
The patient's post office box number. |
Zip+4 |
The 4-digit zip code extension for the patient. |
City |
The patient's city. |
County |
The patient's county of residence. |
Idaho Health District |
This is the health district the patient resides in. This field is auto-populated by IRIS when patient is saved if the patient's address includes an Idaho zip code. |
Phone Number |
The patient's phone number. |
Extension |
The patient's phone number extension. |

The system allows alpha or numeric characters, dashes, periods, forward slashes '/', and pound symbols '#'. |

The system only accepts alpha characters, dashes, apostrophes, and periods. |
Zip |
The system only allows numeric characters. If country is 'United States' and state is 'Idaho', only valid Idaho zips can be entered. |
Zip+4 |
The system only allows numeric characters. |
State |
If a valid Idaho Zip is entered, the state should be Idaho. |
Country |
Country must match with state when country selected is one of United States or a US Territory. |
Phone Number |
The system only allows numeric characters. The area code must be 3 characters if area code is entered. The phone number must be 7 characters if phone number is entered. |
Extension |
The system only allows numeric characters. |
