Assessment Report Screen

 Purpose

The assessment report screen is used to create an assessment of patients current immunization history within four different age categories. An evaluation date is used to determine the cohort for each age category, and Patient Population options are available based upon what type of provider is requesting the assessment.

  

 Functionality

  1. Select a Patient Population option. Different options will be available to you based upon the type of organization you are with. For example, only city or county organizations will see the City or County options. Of note these city or county organizations will also have to be indicated as public organizations.  All of the options whether available or not are described below:

Option

Description

Patients Associated with Selected Site

Selecting this option will return only patients who have been immunized at the selected site.

Patients Associated with Provider

Selecting this option will return only patients who are active within the organization.

Patients Residing in City

Selecting this option will return all patients residing the displayed city.  This report will not exclude patients based on if a patient is active within your organization or not.  This option is only available to city providers, who are also designated as a public organization.

Patients Residing in County

Selecting this option will return all patients residing in the displayed county. This report will not exclude patients based on if a patient is active within your organization or not.  This option is only available to county providers, who are also designated as a public organization.

Patients associated with [Your Organization] or residing in City/County of...

Selecting this option will return a combination of the two previous options. It will return all patients active within your organization or residing within the city/county displayed.

Birth date range

The population will be limited to patients who have a birth date in this range.

Age Range

The population will be limited to patients within this age range as of the evaluation date.

Options for Benchmarking

Select either the Standard Assessment or the Assess Patients with Sufficient Refusal History as Covered option for the report by choosing the appropriate radio button.  Selecting the second option will return an assessment report that counts patients with sufficient refusal comments as being up-to-date.

 

  1. Select your evaluation date criteria. This will be the evaluation date that the assessment uses to determine the cohort for the report. The evaluation date will also limit immunizations considered for the report. No immunizations given after the evaluation date will be included in the evaluation.

  1. Click on the Generate button to begin processing the assessment. If no patients are eligible for the assessment the following message will appear at the top of the screen: "NO PATIENTS MEET CRITERIA".

  2. If there are eligible patients you will be directed to the Assessment Report Status screen where you can follow the progress of your report and view the report once it is finished.

Note:
Refusals of Vaccine:  In order for patient refusals of vaccine to be calculated correctly on assessment and benchmark reports. an appropriate Applies-To-Date must be entered for refusal comments on the Patient Comments tab.

After selecting this option you must select a site within your organization to be used for the report. Selecting this option and not selecting at least one site will generate an error stating that you must select a site to continue.

  1. Select a site from the drop box to the right to be used in the assessment.

  2. Select birth date or age range criteria and your evaluation date.

  3. Generate the report by clicking on the Generate button.

 Additional Notes for City and County Organizations

If you are a county or city public health organization:

  1. Patients associated with your organization means that a patient is selected if he or she is active in your organization.

  2. For County Organizations: Patients residing in a specific county means that a patient is selected if the patient's primary address has a county name which matches, regardless of active status.

  3. For City Organizations: Patients residing in your city means that a patient is selected if the patient's primary address has a city name which matches, regardless of active status.
